For GLWS Members only.  Please complete the following information.  It is important that you complete the 1099 for the season pay schedule as well as the season availability form.  For any services where you wish to request mileage or cartage, please complete the appropriate forms no later than 1 week following a service.   Submissions after this window cannot be guaranteed.  Please also submit your updated contact information and your bio and photo!
