All performing members are required to submit a Federal Form W9 for tax purposes. You may do so by:

1. Download Form W-9 here.
2. Print the form, read the instructions, complete and sign page 1
3. Scan and save only page 1 as a pdf or jpg.
4. Complete all the fields in the form, including attaching the saved W-9 document and submit the link below or submit the form at a rehearsal.
5. NOTE:  Payment cannot be processed without the W9 form on file with the GLWS.
